You will see there are 4 questions for each healthcare topic. As you answer the questions, give yourself a  score using the score chart at the bottom of each page. Add up your total for the 4 questions.

Using the Healthcare Results Sheet, colour in the number of bars in each healthcare practice to show us your score. For example if you got 6 points for Attitude when you did the survey, starting from the centre of the pie, you would colour in 3 bars in the Attitude section. Each bar represent 2 points and 3 x 2 = 6.
